Advocate Health Care builds on Neighborhood Care 

New locations expand access to same-day care in trusted community spaces 

 

Advocate Health Care builds on Neighborhood Care expansion, bringing network to six South Side sites this spring. New locations expand access to same-day care in trusted community spaces

 

Advocate Health Care is expanding its Neighborhood Care network on Chicago’s South Side, with four sites now open and two more set to open later this spring — bringing the total to six community-based locations and marking another milestone in its $1 billion commitment to increase access to care.

 

Designed to bring care closer to where people live, work and gather, Neighborhood Care provides convenient access to same-day, nonemergency care in trusted community settings.

 

“These Neighborhood Care sites are a direct response to what our neighbors told us they need — more access to care, closer to home,” said Michelle Y. Blakely, PhD, president of Advocate Trinity Hospital and Advocate Health Care’s South Side Transformation. “Reaching six sites is an important milestone, and we remain focused on expanding care in ways that build trust and remove barriers.”

 

Four new Neighborhood Care sites are now open, with two additional locations scheduled to open later this spring:

 

Now open: 

•             Neighborhood Care at Salem Care Center

10909 S. Cottage Grove Ave., Front Conference Room

Monday through Friday, 8 a.m. – 4:30 p.m.

•             Neighborhood Care at House of Hope

752 E. 114th St., located through Gate 3

Sundays, 12 – 3 p.m.

•             Neighborhood Care at The Cov

7640 S. Cottage Grove Ave., Suite 114

Monday through Friday, 8 a.m. – 4:30 p.m.

•             Neighborhood Care at Trinity Hospital Professional Office Building

2301 E. 93rd Street, Suite 201

Monday through Friday, 8 a.m. – 6 p.m.


Opening later this spring:

•             Neighborhood Care at El Valor Rey B. Gonzalez Children & Family Center 

3050 E. 92nd St.

Mondays, Wednesdays and Fridays, 7:30 a.m. – 4 p.m.

•             Neighborhood Care at Salud Center

3039 E. 91st St.

Tuesdays and Thursdays, 8 a.m. – 4:30 p.m.

 

These sites build on the momentum of Advocate Health Care’s Neighborhood Care forming a growing network of community-based care across the South Side advancing plans to expand to at least 10 locations.

 

In addition to these community-based sites, Advocate Health Care also offers Neighborhood Care services in a mobile setting:

•             Mobile Neighborhood Care at Compassion Baptist Church 

2650 E. 95th St. 

Wednesdays, 10 a.m. – 3 p.m.

 

“By bringing care into spaces the community already knows and trusts, we’re making it easier for people to get the care they need — supporting them in staying well, managing chronic conditions and connecting to care early,” Dr. Blakely said. “These neighborhood care sites reflect our commitment to improving the health and wellness across the South Side for the long term.”

 

Neighborhood care sites are designed to offer the right level of care in convenient community settings, making it easier to get care close to home—offering same-day services and connections to a variety of resources to meet health and social needs, along with financial counselors who can help patients understand coverage and costs.
